About Us
Packard Community Clinic is a
‘health
care home’ for insured and uninsured Washtenaw County
community members with a national model of effective health
care
delivery. In addition to fulfilling its mission to provide health care
for all, regardless of ability to pay, the founders, Board, and staff
at Packard Community Clinic have a vision for
excellence in primary health care that fully integrates mental health
care and health promotion.
- Nearly 50% of Packard’s
patients are
insured and over 50% are uninsured.
- To improve access to care, Packard offers
evening and weekend hours.
- Staff and patients are racially and
culturally
diverse;
- One third of the patients are African
American,
- One third European American, and
- One third Latino, Arabic or Asian.
Primary
Health Care
Packard Community Clinic is a growing family
practice that offers a full range of primary health care services.
- In fiscal year 2005-2006, 6,053 patients
were
seen.
- Of these, 1,987 were new patients.
- Total visits for the year were over
19,000.
Mental Health Care
In collaboration with the Washtenaw County
Health
Organization and others, Packard Community Clinic offers fully
integrated psychiatric, psychological, and counseling services.
Packard’s mental health team recorded 840 social work visits
and 240 psychiatric visits at no cost to the patient.
Health Promotion and Chronic Disease
Management
Packard Community Clinic’s
collaboration with the Washtenaw County Health Plan and others has
fostered the development of high quality health promotion and chronic
disease
management programs.
- The registered dietician recorded 321
nutritional counseling sessions.
- 88 patients accessed chronic disease case
management services.
- Health education and wellness programs
were
attended regularly by over 150 community members.
Our Transition
After 33 years of service, patients, staff,
colleagues, and community members celebrated Jerry and Julie
Walden’s tremendous vision, service, and retirement. They
remain committed to Packard Community Clinic.
Packard Community Clinic’s Medical
Director, Raymond Rion,
MD is a highly regarded family physician who came to
Packard Clinic in 2003 from the University of Michigan. Executive
Director, Kimberly
Kratz, MSW, MPH has a solid foundation in nonprofit
organizational leadership and public health with underserved
communities.
